To promote the mental, physical and emotional well-being of individuals and families in Northeast Florida by providing easily accessible, culturally competent, quality-based clinical services.

Our History

Community Rehabilitation Center, Inc. Was founded in 1993 in response to the immense community need for a comprehensive mental health program for the minority population living on Jacksonville’s Northside. Since the agency’s beginning,CRC has significantly expanded servíceš to also provide a comprehensive, multicultural mental health program that serves more than 300 clients daily. In addition to mental health, CRC also provides services to address the substance abuse, HIV, and primary health care needs of the community.

Our Values

CRC is a faith based agency that believes people who are in need of such services as mental health or substance abuse counseling need a holistic approach that involves reconnection to their community, evidenced based interventions that are specifically designed to meet their unique needs and diverse, highly skilled, knowledgeable staff that fosters hope, faith and commitment.

Therefore, our programs are designed to incorporate the individual, their families and support network, neighborhoods, community organizations and institutions into a plan of action to ensure each person we serve has the best opportunity for change.

    • Aim to prevent and reduce the onset of substance abuse and transmission of HIV/VH among racial and sexual minority youth, young adults and high populations.
    • We are a registered test site through the Florida Department of Health.
  • Transportation

    • Provided daily to and from CRC
  • Emergency Food Pantry

    • Food distribution to the surrounding community made possible through partnerships and local food banks.
  • Individual and Group Counseling

    • Post-Traumatic Stress Disorder: Treatment for Veterans and non-Veterans living through traumatic events.
    • Anger Management – Managing anger before it controls you.
    • Diabetes & Depression – Coping with Diabetic Stress
    • Children’s Behavioral Counseling
    • Violence Prevention Counseling
  • Substance Abuse Treatment

    • Prevention Level 1, Intervention, General Outpatient and Aftercare
    • Medication Assisted Treatment for Opiod Use Disorder (OUD)
      • We utilize an individualized patient-centered approach to improve treatment engagement so patients can implement positive long-term changes in their lives and achieve sustained addiction remission.
